• Full-time, Day shifts (8-hour shifts, Monday through Friday)

Key Responsibilities

• Provide skilled occupational therapy services to patients in a home health setting.
• Conduct patient assessments and develop individualized treatment plans.
• Document patient progress and maintain accurate records using electronic charting systems such as EPIC and Rover.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Travel within designated geographic area to visit multiple patients per day, minimizing drive time through efficient scheduling.

Required Qualifications

• Minimum of 1 year of skilled home health experience as an Occupational Therapist.
• Valid and pending state license accepted; active license required prior to start.
• BCLS certification required.
• Ability to work independently and manage a caseload of up to six patients per day.
• Reliable personal transportation to travel within the area.

Preferred Experience / Skills

• Experience with Oasis charting and familiarity with home health documentation systems.
• Comfortable working in a fast-paced environment with scheduling flexibility.
