• Full-time, Day shifts, 8-hour shifts (Monday through Friday)

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Seaside and surrounding areas.
• Assess patient conditions, develop, and implement individualized treatment plans.
• Document patient progress using EPIC and Rover systems, adhering to facility charting protocols.
• Visit up to six patients per day, maintaining optimized travel routes to minimize drive time.
• Collaborate with interdisciplinary teams to ensure quality patient care and achieve productivity goals.
• Maintain compliance with all facility and licensing regulations, including COVID vaccination requirements.

Required Qualifications

• Valid and pending state licensure as a Physical Therapist.
• Minimum of 1 year of skilled home health experience.
• Current BCLS certification.
• Own transportation to travel within assigned radius.

Preferred Experience / Skills

• Previous experience with Oasis documentation system.
• Ability to work independently and efficiently in a home health setting.
• Strong organization skills to manage multiple patient visits and travel efficiently.
