Job Title

Schedule / Job Type

• Full-time, Day shifts, 5 days per week

Key Responsibilities

• Provide inpatient case management services within an acute hospital setting.
• Collaborate with medical teams to develop and implement patient care plans.
• Support patients through assessments, discharge planning, and resource coordination.
• Document patient interactions and care details accurately using Epic charting system.
• Float within the hospital or to affiliate facilities as needed to meet operational needs.

Required Qualifications

• Valid State License as LMSW or LCSW (pending license acceptance is acceptable with a 10-12 week processing time).
• Minimum of 1 year of acute care experience.
• Must have experience with Epic charting.
• Must be willing to work major holidays (Thanksgiving, Christmas, New Year).
• Must be at least 50 miles from the facility to be considered.

Preferred Experience / Skills

• Previous inpatient or hospital social work experience.
• Strong interdisciplinary communication skills.
• Ability to work independently and within a team in a fast-paced environment.
