Job Title

Schedule / Job Type
• Full-time, up to 40 hours per week, in-home therapy services

Key Responsibilities
• Provide in-home physical therapy services to patients in a home health and hospice setting
• Assess patient needs and develop personalized treatment plans to promote independence and safety
• Collaborate effectively with a multidisciplinary team to ensure comprehensive patient care
• Maintain accurate documentation of treatments and patient progress
• Work independently while adhering to all state licensure and safety standards

Required Qualifications
• Valid Vermont license as a Physical Therapist
• Valid driver’s license and reliable vehicle for home visits
• Ability to work independently and in a team environment

Preferred Experience / Skills
• Prior experience in a home care or hospice setting is preferred
