- Full-time, Days shifts, Self-scheduling, 8:00 AM - 4:30 PM
Key Responsibilities
- Provide outpatient, acute inpatient, skilled nursing, and home health physical therapy services.
- Manage an average of 5 patients per day, including evaluations and visits, with flexibility depending on patient needs.
- Utilize skills in OASIS documentation, with Lymphedema certification considered a plus.
- Collaborate with Rehab Aides and supervised by licensed PTs to deliver comprehensive care.
- Follow department-specific orientation including computer-based training and shadowing.
Required Qualifications
- Valid BLS certification.
- Experience with OASIS documentation.
- Ability to treat geriatric patients with diagnoses such as joint replacements, stroke, CHF/COPD, and Parkinson’s.
- Willingness to work in home health settings and cover assigned areas.
Preferred Experience / Skills
- Lymphedema certification.
- Prior experience in outpatient or home health rehab.
