Schedule / Job Type

• Full-time, Day shifts, 4x10-hour shifts per week (Wednesday to Saturday)

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across multiple counties
• Assess patient needs and develop appropriate treatment plans
• Document patient progress and treatment outcomes accurately
• Collaborate with interdisciplinary teams to optimize patient care
• Travel within designated counties, with mileage reimbursement as applicable
• Adhere to all safety and compliance protocols, including initial and ongoing competency assessments

Required Qualifications

• Valid state license as a Physical Therapist, or pending license accepted
• Minimum of 1-2 years of home health experience preferred
• Basic life support (BLS) certification required
• Recent EPIC experience preferred but not mandatory
• Ability to pass Cardiac Dysrhythmia and Moderate Sedation tests during orientation
• Willingness to float within scope to meet facility needs

Preferred Experience / Skills

• Prior experience with SRHS or similar health systems
• Familiarity with travel compliance and RTO scheduling
• Strong documentation and communication skills
