Key Responsibilities
- Provide home health nursing visits to patients, traveling between patient homes within the community.
- Manage and complete assessments using OASIS data collection.
- Administer outpatient care and coordinate with hospice services as needed.
- Adjust schedules as necessary to accommodate patient needs and visit changes.
- Maintain accurate documentation within the EMR system (HealthWyse/Careficient).
- Participate in assigned call rotations for emergencies.
Required Qualifications
- Valid driver’s license in good standing.
- BLS certification.
- Proven experience with OASIS assessments.
- Prior home health and hospice care experience.
- Ability to travel regularly and manage a dynamic schedule.
- Familiarity with EMR systems, specifically HealthWyse (Careficient).
Preferred Experience / Skills
- Experience working in outpatient settings.
- Comfort working independently in patients' homes.
- Strong adaptability to schedule changes and emergency call requirements.
