Key Responsibilities

• Provide social work services in a home health and hospice setting, supporting terminally ill patients and their families.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage complex symptom management and end-of-life care education.
• Participate in goals of care discussions and coordinate patient and family support.
• Document patient interactions and care activities using the Net Smart EMR system.
• Collaborate with the clinical team to ensure high-quality, compassionate end-of-life care.

Required Qualifications

• Master’s Degree in Social Work (MSW).
• Vermont LMSW licensure.
• Minimum of 2 years of relevant clinical experience in social work.
• Experience with Epic EMR (within the past 24 months).
• BLS certification.

Preferred Experience / Skills

• Prior home health or hospice social work experience.
• Strong assessment and critical thinking skills in end-of-life care.
• Ability to teach and support patients and caregivers in complex symptom management.
• Valid driver’s license and reliable transportation for patient visits in winter conditions.
