Key Responsibilities
• Conduct comprehensive patient and caregiver assessments to develop individualized care plans aligned with evidence-based practices and regulatory standards.
• Serve as Case Manager, actively managing patient caseloads and visit schedules to meet productivity expectations.
• Complete and document OASIS assessments accurately and timely, remaining current on documentation requirements.
• Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, and quality patient care in compliance with regulations.
• Stay current on high-tech skills such as IV therapy, wound VAC, complex wound care, and infection control standards.
• Collaborate with LPNs and multidisciplinary teams to ensure care plan adherence and delegate tasks appropriately.
• Evaluate care effectiveness by reviewing patient responses and updating care plans to meet evolving needs.
• Maintain precise and comprehensive documentation to ensure seamless communication within the healthcare team.
• Educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions.
• Identify and document interdisciplinary needs like physical therapy and social work, facilitating timely communication with relevant providers.
• Complete agency, state, and federal forms, including Medicare notices, in a timely and accurate manner.
• Conduct patient visits efficiently, communicating scheduling needs and follow-up requirements timely.
• Assess patient needs for HCA services, develop ongoing care plans, and provide supervision in accordance with regulations.
• Report patient incidents promptly to Quality Improvement staff.
• Coordinate discharges and transfers, ensuring all paperwork and patient information are properly processed.
• Collaborate with physicians, providers, and vendors to support safe and effective care delivery.
• Participate in peer reviews, audits, and case conferences to foster continuous improvement and communication.
• Advocate proactively for patient care needs, ensuring personalized and sensitive treatment.
• Conduct initial and ongoing assessments within IDT meetings, developing and updating individualized care plans regularly.
• Stay informed about pain management, symptom control, high-tech interventions, and infection control standards.
• Provide patient and caregiver education to promote informed decision-making and self-care.
• Coordinate with healthcare team members to ensure continuity and quality of care for every patient.

Required Qualifications
• Current State Registered Nurse (RN) license
• Experience with OASIS assessments in a home health setting
• Knowledge of federal and state healthcare regulations and standards
• Ability to perform high-tech nursing skills such as IV therapy and wound care
• Strong documentation and communication skills

Preferred Experience / Skills
• Previous home health or community health experience
• Familiarity with interdisciplinary team coordination and case management
• Proficiency in using healthcare documentation systems and electronic health records
