Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility.
• Develop and implement treatment plans to improve patient mobility and function.
• Document patient progress and communicate with healthcare team members.
• Ensure compliance with all state licensing requirements and facility policies.
• Maintain accurate and timely patient records.

Required Qualifications

• Arizona licensure as a Physical Therapist.
• Proven experience working in a skilled nursing or similar setting.
• Strong knowledge of physical therapy principles and practices.
• Excellent communication and patient care skills.
• Ability to work independently and as part of a healthcare team.

Preferred Experience / Skills

• Prior experience in a long-term care or rehabilitation facility.
• Knowledge of electronic health records (EHR) systems.
