Job Title

• LVN / LPN - Long-Term Acute Care (LTAC)

• Baltimore, MD

Schedule / Job Type

• Full-time, night shift, 3 shifts per week, 12-hour shifts

Key Responsibilities

• Provide direct patient care to LTAC residents across multiple units, including subacute rehab, long-term care, and specialty units
• Float within hospital units as needed, delivering consistent and compassionate nursing care
• Follow facility protocols for infection control, safety, and documentation
• Recognize and respond to patient needs, monitor vital signs, and administer medications as ordered
• Document patient progress and collaborate with interdisciplinary teams to develop care plans
• Adhere to strict attendance and punctuality policies, including working every other holiday

Required Qualifications

• Valid Maryland or Compact LVN/LPN license at the time of submission
• Minimum of 1 year of nursing experience in a hospital or long-term care setting
• Current ACLS and BLS certifications
• Ability to work night shifts and accommodate floating responsibilities
• Excellent organizational and communication skills

Preferred Experience / Skills

• Experience caring for ventilator patients and with trach care
• Familiarity with subacute rehab, memory care, or pulmonary units
• Comfortable working in a fast-paced, high-acuity environment
