Key Responsibilities

• Perform comprehensive assessments of residents’ physical abilities, movement limitations, and pain levels.
• Develop and implement individualized treatment plans based on evaluations, medical history, and patient goals.
• Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, and other therapists to coordinate comprehensive care.
• Monitor patient progress, adjust therapy plans as needed, and accurately document all care provided.
• Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use.
• Ensure compliance with all regulatory requirements, including Medicare/Medicaid documentation and facility policies.
• Supervise physical therapist assistants and aides as required.
• Maintain a clean and safe work environment.

Required Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist (PT).
• CPR certification (preferred or required based on facility requirements).
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Preferred Experience / Skills

• Experience in a skilled nursing or geriatric rehabilitation setting preferred.
• Familiarity with inpatient, long-term care, or short-term rehabilitation environments.
