Key Responsibilities
- Perform comprehensive assessments of residents’ physical abilities, movement limitations, and pain levels.
- Develop and implement individualized treatment plans based on evaluations, medical history, and patient goals.
- Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
- Collaborate with physicians, nursing staff, and other therapists to coordinate comprehensive care.
- Monitor patient progress, adjust therapy plans as needed, and accurately document all care provided.
- Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use.
- Ensure compliance with all regulatory requirements, including Medicare/Medicaid documentation and facility policies.
- Supervise physical therapist assistants and aides as required.
- Maintain a clean and safe work environment.
Required Qualifications
- Graduate of an accredited Physical Therapy program.
- Current state license to practice as a Physical Therapist (PT).
- CPR certification (preferred or required based on facility requirements).
- Excellent communication, documentation, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Preferred Experience / Skills
- Experience in a skilled nursing or geriatric rehabilitation setting preferred.
- Familiarity with inpatient, long-term care, or short-term rehabilitation environments.
