Key Responsibilities
- Provide in-home occupational therapy services to home care and hospice patients
- Maximize patient independence and safety within the home environment
- Function as a collaborative member of a multidisciplinary care team
- Work independently to manage a caseload of home-based patients
Required Qualifications
- Active Vermont Occupational Therapist licensure
- Valid driver's license and reliable personal vehicle
- Ability to work independently while collaborating effectively with a multidisciplinary team
Preferred Experience / Skills
- Prior experience in a home care or hospice setting
