Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs
• Deliver therapeutic interventions and monitor patient progress toward established goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the clinical team to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist license required; permanent state licensure required (compact licensure alone is not sufficient)
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing requirements prior to assignment start
• Candidates must not have worked at this facility in a permanent or PRN capacity within the past year

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Strong documentation skills and familiarity with outpatient EMR systems
