Key Responsibilities

• Develop individualized care plans in collaboration with the patient's physician, patient, and family or primary caregivers upon admission; periodically review and recertify care plans as appropriate.
• Coordinate patient care through interdisciplinary communication and case conferences.
• Provide ongoing assessment of patients' clinical, functional, and emotional status; communicate significant changes to the physician and care team colleagues.
• Document evaluations, patient responses, and progress toward established goals.
• Teach patients and caregivers care techniques and end-of-life symptom management as appropriate.

Required Qualifications

• Active Registered Nurse licensure in both Vermont and New Hampshire, or an unencumbered compact nursing license.
• Graduate of an accredited school of nursing.

Preferred Experience / Skills

• Experience in Medical/Surgical or Oncology nursing.
• Prior hospice care experience.
