Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs with the goal of restoring function and minimizing disability following disease or injury
• Conduct initial admission and evaluation visits, documenting clinical findings, skilled interventions, goals, and discharge plans
• Complete and submit OASIS scores within established timelines to the assigned Clinical Team Manager
• Supervise Physical Therapy Assistants as applicable
• Help patients achieve maximum performance and function within the limits of their capabilities
• Perform additional duties consistent with skills, training, and the goals of the position

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of one year of Home Health experience
• Proficiency in evaluating and treating patients under medically prescribed physical therapy programs
• Familiarity with OASIS documentation and reporting requirements
• Ability to work independently in a field-based, home health environment

Preferred Experience / Skills

• Experience supervising Physical Therapy Assistants
• Familiarity with the service territory including Milpitas, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy, and surrounding areas
• Strong documentation and clinical communication skills
