Key Responsibilities
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Develop and implement personalized treatment plans based on evaluation findings.
- Utilize a variety of therapeutic techniques and modalities to facilitate patient recovery.
- Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to optimize patient care.
- Educate patients and their families on rehabilitation goals, techniques, and home exercise programs.
- Maintain accurate and timely documentation of patient assessments and treatments.
Required Qualifications
- Valid and unrestricted Physical Therapist license required.
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
- Confirmed SNF post-graduate experience required.
- PCC and/or NetHealth proficiency required.
- Strong interpersonal and communication skills.
- Ability to work effectively in a team-oriented environment.
Preferred Experience / Skills
- Experience working in a high-volume, fast-paced skilled nursing or long-term care environment.
- Familiarity with current industry best practices and regulatory requirements in SNF/LTC settings.
