Key Responsibilities

• Provide medical social services to hospice patients and their families through the final stages of illness
• Conduct home visits across a multi-county service area, including patients residing in private homes, skilled nursing facilities, assisted living facilities, and adult foster homes
• Collaborate with an interdisciplinary hospice team to deliver coordinated care including pain and symptom management, counseling, and spiritual support
• Offer bereavement support and connect patients and families with appropriate community resources
• Participate in on-call coverage responsibilities as part of a 24-hour hospice care model

Required Qualifications

• Active Oregon LMSW (Licensed Master Social Worker) or CSWA (Clinical Social Work Associate) licensure required
• Healthcare Provider Level BLS certification required
• Valid driver's license and personal auto liability/property damage insurance as required by Oregon law
• Must maintain personal vehicle in good working order for use in patient home visits

Preferred Experience / Skills

• Oregon LCSW (Licensed Clinical Social Worker) licensure
• Prior experience as a social worker in a hospital or medical setting
• Familiarity with hospice, palliative care, or end-of-life care environments
