Schedule / Job Type
Contract | 13 Weeks | Full-Time | Onsite | Days | Monday–Friday, 8:00 AM–4:30 PM | No weekend requirement | Holiday coverage may be required as needed
Key Responsibilities
- Conduct comprehensive psychosocial assessments and independently develop, implement, and adjust individualized treatment and discharge plans
- Manage a full caseload of moderate to high complexity patients, including those with co-occurring disorders, psychosocial instability, and discharge barriers
- Provide crisis intervention and evidence-based therapeutic services across individual, group, and family settings
- Lead discharge planning efforts, proactively identifying and resolving barriers related to housing, insurance, legal status, and access to care
- Coordinate complex transitions of care, ensuring continuity across inpatient, outpatient, and community-based services
- Independently manage court coordination and legal processes, ensuring accuracy and timeliness, while maintaining high-quality clinical documentation that reflects decision-making and patient progress
Required Qualifications
- Master's Degree in Social Work required
- Active LCSW licensure required
- BLS certification required
- CPI certification required
- Proficiency with Epic electronic health records required
- Minimum 2 years of clinical social work experience required
- New graduates are not eligible for this position
Preferred Experience / Skills
- Prior travel social work or contract assignment experience preferred
- Experience working with patients with co-occurring disorders and complex psychosocial needs
- Familiarity with regulatory, legal, and accreditation standards including mandated reporting and patient rights
