Key Responsibilities
- Provide physical therapy services to patients in a home health setting
- Complete and manage OASIS assessments in accordance with home health requirements
- Develop and implement individualized treatment plans to support patient recovery and functional goals
- Document patient progress and maintain accurate clinical records
- Travel to patient homes within the assigned service area; mileage reimbursed at the IRS rate
Required Qualifications
- Active Physical Therapist license; Virginia license required to be in hand at time of submittal or proof of compact license
- Minimum 6 months to 1 year of recent home health experience required
- OASIS experience required
- Specialty experience within the last year required
- One supervisory reference required at time of submittal
- Local candidates are welcome and accepted
Preferred Experience / Skills
- Experience managing a varied home health caseload independently
- Strong documentation and time management skills in a field-based environment
