Key Responsibilities

• Observe and report symptoms and conditions of residents; notify MD/NP and family members of any changes in resident status
• Administer medications as prescribed by physician and perform treatments and assessments as required
• Maintain accurate documentation including nurses notes, care plans, medication and treatment records, and transcribe physician written and telephone orders
• Perform resident admissions and discharges; assist with completion of nursing history, assessments, MDS, and care plans
• Directly supervise CNAs, prepare and coordinate CNA assignments, and assist in orientation and supervision of nursing staff
• Assume Charge Nurse responsibilities when necessary; coordinate with MDs for rounds, tests, labs, x-rays, and medical appointments

Required Qualifications

• Current, active LPN license in the State of Massachusetts
• Graduate of an accredited school of nursing
• Current CPR/AED Certification required
• Ability to recognize changes in resident condition and take appropriate action
• Ability to read, write, and speak English in a legible and understandable manner
• Ability to meet physical requirements including lifting up to 50 lbs, pushing/pulling up to 40 lbs, and standing for up to 4 hours per day

Preferred Experience / Skills

• Prior experience in a long-term care or post-acute skilled nursing environment
• Experience with MDS completion and care plan development
• Prior charge nurse or supervisory experience in a nursing setting
