Key Responsibilities
- Observe and report symptoms and conditions of residents; notify MD/NP and family members of any changes in resident status
- Administer medications as prescribed by physician and perform treatments and assessments as required
- Maintain accurate documentation including nurses notes, care plans, medication and treatment records, and transcribe physician written and telephone orders
- Perform resident admissions and discharges; assist with completion of nursing history, assessments, MDS, and care plans
- Directly supervise CNAs, prepare and coordinate CNA assignments, and assist in orientation and supervision of nursing staff
- Assume Charge Nurse responsibilities when necessary; coordinate with MDs for rounds, tests, labs, x-rays, and medical appointments
Required Qualifications
- Current, active LPN license in the State of Massachusetts
- Graduate of an accredited school of nursing
- Current CPR/AED Certification required
- Ability to recognize changes in resident condition and take appropriate action
- Ability to read, write, and speak English in a legible and understandable manner
- Ability to meet physical requirements including lifting up to 50 lbs, pushing/pulling up to 40 lbs, and standing for up to 4 hours per day
Preferred Experience / Skills
- Prior experience in a long-term care or post-acute skilled nursing environment
- Experience with MDS completion and care plan development
- Prior charge nurse or supervisory experience in a nursing setting
