Contract | Full-Time | Days (Monday–Friday, 8:00 AM – 5:00 PM) | Field-Based / Travel Required

Key Responsibilities

• Manage an active patient case load within a home health setting across assigned service areas
• Conduct patient visits throughout the day, traveling between patient homes in the assigned coverage area
• Perform clinical assessments, care planning, and coordination of services for home health patients
• Document patient care and clinical notes using HomeCare HomeBase (HCHB) electronic charting system
• Collaborate with interdisciplinary team members to ensure continuity and quality of patient care

Required Qualifications

• Active Registered Nurse (RN) license in the state of North Carolina (temporary or pending licenses considered)
• Minimum 1 year of case management experience in a home health setting
• Experience with HomeCare HomeBase (HCHB) charting system
• Prior travel nursing experience required
• Must have a personal vehicle and valid driver's license for daily patient travel
• Ability to work some holidays as needed

Preferred Experience / Skills

• Strong organizational and time management skills suited for an independent, field-based role
• Experience managing a high-volume home health case load across multiple locations
• Familiarity with home health regulatory and compliance standards
