Contract | Days (5x8 Hours, 8:00–17:00) | Onsite

Key Responsibilities

• Provide physical therapy evaluation and treatment services to patients in a home health or facility setting
• Develop and implement individualized treatment plans in accordance with physician orders and patient needs
• Document patient progress, treatment plans, and outcomes using electronic charting systems
• Complete required orientation modules within 30 days of start date
• Maintain compliance with scheduling expectations, including availability to work some holidays

Required Qualifications

• Graduate degree from an accredited Physical Therapy Program approved by the APTA
• Current and unrestricted Physical Therapy licensure (temporary or pending licenses considered)
• Minimum of one year of physical therapy experience; home health or facility setting preferred
• Current CPR certification (ARC accepted)
• Travel experience required
• Two supervisory references required

Preferred Experience / Skills

• Experience with HomeCare HomeBase charting software
• Prior home health experience strongly preferred
