Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, develop, and implement individualized treatment plans based on patient needs
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with interdisciplinary team members to ensure continuity and quality of patient care
• Adhere to all credentialing, compliance, and documentation requirements prior to and throughout the assignment

Required Qualifications

• Active Physical Therapist license required; compact license accepted at start but permanent state licensure must be obtained promptly
• Prior experience in an outpatient physical therapy setting
• Must meet all contract credentialing and quality assurance requirements before assignment begins
• Candidates who have worked permanently or PRN at this facility within the last year are not eligible for this assignment

Preferred Experience / Skills

• Experience working in a high-volume outpatient therapy environment
• Strong documentation skills and ability to adapt to facility-specific EMR or documentation platforms
• Demonstrated ability to manage a full patient caseload efficiently
