Licensed Practical Nurse – Long Term Care / Post-Acute Skilled Nursing Rehab

Contract | Days | Monday through Sunday (no block schedules)

Key Responsibilities

• Provide direct patient care on a Post-Acute Skilled Nursing Rehab unit serving a patient population aged 60+, with a nurse-to-patient ratio of 1:14
• Perform unit-specific procedures including phlebotomy, EKGs, patient transports, nebulizer-to-inhaler conversions, and starting IVs
• Document patient care and administer medications using EPIC
• Float on an occasional, as-needed basis to Long-Term Care and Memory Care units
• Work a rotating schedule that includes a minimum of every other weekend and coverage adjusted to meet unit and patient care needs; must be available to work all holidays
• Operate IV pumps and coordinate with ancillary support staff including LNA/CNA/PCT and unit clerks (day shift)

Required Qualifications

• Active Vermont (VT) LPN license required at time of submission
• Minimum of 1 year of clinical experience in specialty (Post-Acute / Skilled Nursing)
• EPIC experience within the past 2 years is required and non-negotiable
• Prior travel nursing experience required
• Must be able to pass a 7-year background check (nationwide, county court searches, FACIS 3, and sex offender) completed within 18 months of start date; FACIS 3 and VCIC required within 30 days of start
• Reliable personal or rental vehicle required; public transportation is not accessible in this area — vehicles should be equipped with all-season or snow tires from November through April

Preferred Experience / Skills

• Dementia-capable care certification preferred
• IV Certification preferred for LPNs
• Experience working with skilled nursing rehab patient populations (diagnoses including post-acute skilled nursing rehab)
