Contract | Days | Monday–Friday, 8:00 AM–5:00 PM | No weekends or holidays unless pre-approved | 13 Weeks

Key Responsibilities

• Provide home health physical therapy services to patients within the greater Houston region
• Complete the expected 32–34 visit points per week, with drive time factored into scheduling
• Contact the appropriate team to report all missed visits or changes in evaluation frequency, or when unable to reach a patient
• Document all patient care using Wellsky charting software
• Maintain a clean, professional appearance including scrubs and closed-toe shoes; facial jewelry limited to small nose stud and earrings only

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• BLS and First Aid certification required
• Minimum of 2 years of recent licensed PT experience (private duty experience does not qualify; new graduates are not eligible)
• Home health experience required; this may be a first travel assignment provided home health experience requirements are met

Preferred Experience / Skills

• Prior experience with Wellsky or similar home health documentation platforms
• Strong time management skills with the ability to manage a high-volume visit schedule independently
• Familiarity with home health visit point-based productivity systems
