Contract | Days | 5 x 8-hour shifts | Expected 40 hours/week

Key Responsibilities

  • Provide direct physical therapy services to patients in an outpatient setting
  • Evaluate, develop, and implement individualized treatment plans based on patient needs
  • Complete required clinical documentation accurately and in a timely manner
  • Collaborate with interdisciplinary team members to support patient progress and outcomes
  • Maintain compliance with credentialing and facility-specific documentation requirements prior to assignment start

Required Qualifications

  • Active Physical Therapist license (compact license accepted; permanent state license must be obtained promptly upon assignment start)
  • Experience in an outpatient physical therapy setting
  • Ability to meet facility credentialing and contract QA requirements

Preferred Experience / Skills

  • Prior experience working in a high-volume outpatient rehabilitation environment
  • Strong documentation skills and familiarity with electronic medical records
