Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver therapeutic interventions in accordance with established clinical standards and care protocols
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to support patient progress and outcomes

Required Qualifications

• Active Physical Therapist license required
• Experience in an outpatient physical therapy setting
• Must obtain a permanent state license prior to or immediately upon assignment start
• Must meet credentialing and contract quality assurance requirements

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient rehabilitation environment
• Strong documentation skills and familiarity with electronic medical records
