Contract | 13 Weeks | Full-Time | Days | Monday–Friday 8:30 AM–5:00 PM with alternating weekends | On-call rotation required (approximately 1–2 weeks per month, weekday evenings 5:30 PM–8:30 AM; phone triage-based, home visits rare)

Key Responsibilities

• Provide skilled nursing care to patients in a home health setting, managing a caseload of approximately 30 points per week
• Perform Start of Care (SOC), Resumption of Care (ROC), recertification, revisit, and OASIS discharge assessments
• Complete accurate and timely clinical documentation using an electronic medical records system
• Participate in on-call and phone triage rotation, approximately 1–2 weeks per month during evening and overnight hours on weekdays
• Deliver patient care with professionalism, compassion, and a strong focus on customer service excellence
• Manage time effectively to meet productivity requirements and maintain reliable, dependable patient visit schedules

Required Qualifications

• Active Registered Nurse (RN) license — must be in hand at time of submission
• Minimum of 2 years of home health nursing experience
• Homecare Homebase and OASIS experience within the last year — required for consideration
• Experience with on-call and phone triage responsibilities
• Strong time management, organizational, and communication skills

Preferred Experience / Skills

• Experience with Wellsky EMR
• Wound care experience
