Key Responsibilities

• Perform physical therapy visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits in patients' homes
• Develop and implement individualized physical therapy plans of care based on patient needs and physician orders
• Provide skilled interventions including gait training, balance training, therapeutic exercise, mobility training, and fall prevention
• Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
• Complete all required documentation accurately and timely in Homecare Homebase (HCHB), ensuring compliance with Medicare Conditions of Participation and applicable payer requirements
• Collaborate with an interdisciplinary care team including RNs, PTAs, OTs, and STs, and provide clinical oversight of Physical Therapist Assistants as permitted under Colorado law

Required Qualifications

• Current, unrestricted Colorado Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver's license and reliable personal transportation
• Ability to practice independently in a home-based setting
• Proficiency in or willingness to learn Homecare Homebase (HCHB)
• Compliance with HIPAA and all applicable privacy and security requirements

Preferred Experience / Skills

• Prior home health experience
• Familiarity with Medicare documentation standards and home health regulations
• Experience working with geriatric and medically complex patient populations
