Key Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care; implement, evaluate, and revise nursing interventions based on patient needs and clinical circumstances
• Administer medications and treatments safely and accurately per provider orders; recognize and respond to changes in patient condition with appropriate escalation of care
• Provide patient and family education related to care, medications, and discharge needs, delivering culturally responsive, patient-centered care throughout
• Collaborate with providers and interdisciplinary team members to coordinate care, facilitate referrals, follow-up care, and continuity of services
• Document assessments, interventions, and outcomes accurately and timely in the electronic health record; advocate for patients in navigating the healthcare system
• Participate in quality and performance improvement activities; adhere to regulatory, safety, and infection prevention requirements within nursing scope of practice

Required Qualifications

• Active, unrestricted Alaska Registered Nurse (RN) license required at time of submittal
• Graduate of an accredited school of nursing
• Current Basic Life Support (BLS) certification required
• High school diploma or equivalent required
• Ability to apply independent nursing judgment consistent with Alaska Board of Nursing scope of practice standards

Preferred Experience / Skills

• Bachelor's degree in Nursing (BSN) from an accredited program
• Case Management certification
• Experience in an ambulatory or outpatient clinic setting
• Experience with care coordination, referral management, and electronic health record documentation
