Key Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care; implement, evaluate, and revise nursing interventions based on patient needs and clinical circumstances
- Administer medications and treatments safely and accurately per provider orders; recognize and respond to changes in patient condition with appropriate escalation of care
- Provide patient and family education related to care, medications, and discharge needs, delivering culturally responsive, patient-centered care throughout
- Collaborate with providers and interdisciplinary team members to coordinate care, facilitate referrals, follow-up care, and continuity of services
- Document assessments, interventions, and outcomes accurately and timely in the electronic health record; advocate for patients in navigating the healthcare system
- Participate in quality and performance improvement activities; adhere to regulatory, safety, and infection prevention requirements within nursing scope of practice
Required Qualifications
- Active, unrestricted Alaska Registered Nurse (RN) license required at time of submittal
- Graduate of an accredited school of nursing
- Current Basic Life Support (BLS) certification required
- High school diploma or equivalent required
- Ability to apply independent nursing judgment consistent with Alaska Board of Nursing scope of practice standards
Preferred Experience / Skills
- Bachelor's degree in Nursing (BSN) from an accredited program
- Case Management certification
- Experience in an ambulatory or outpatient clinic setting
- Experience with care coordination, referral management, and electronic health record documentation
