Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaints in accordance with RN scope of practice and facility policies.
• Develop, implement, and continuously evaluate individualized plans of care in coordination with patients, families, and the inter-professional team.
• Initiate and carry out nursing interventions, procedures, and therapies with clinical autonomy; recognize, respond to, and communicate changes in patient condition in a timely manner.
• Facilitate admission, transfer, and discharge activities, including discharge teaching and coordination with Case Management.
• Delegate tasks appropriately and monitor the accuracy and completeness of care provided by other team members.
• Actively participate in unit-based quality improvement activities, including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification
• Associate Degree in Nursing (ADN or ASN) required
• Ability to communicate and collaborate effectively with all members of the care team, both verbally and in writing
• Proficiency with or ability to be trained on computer systems and clinical applications
• Ability to assess age-specific patient safety needs and implement appropriate safety measures

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting
• Bachelor of Science in Nursing (BSN)
