Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, and risk factors in accordance with RN scope of practice and facility policies
• Develop, implement, and continuously evaluate individualized plans of care in coordination with patients, families, and the inter-professional team
• Administer patient care therapies and procedures with clinical autonomy; recognize, respond to, and communicate changes in patient condition in a timely manner
• Facilitate admission, transfer, and discharge activities including discharge teaching, coordination with Case Management, and execution of discharge orders
• Delegate tasks appropriately and monitor accuracy and completeness of care provided by other team members
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification required
• Associate Degree in Nursing (ADN or ASN) required
• Ability to communicate and collaborate effectively with all members of the care team
• Proficiency with or ability to be trained on relevant computer systems and clinical applications

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting
• Bachelor of Science in Nursing (BSN)
