Key Responsibilities
- Provide skilled nursing care to home health patients within the assigned service area, managing approximately 5–6 patients per day
- Conduct in-home patient visits throughout the Franklin/Macon County area, requiring use of a reliable personal vehicle
- Document all patient care activities using the HCHB documentation system
- Participate in an on-call rotation approximately every 6–7 weeks, covering one week of after-hours on-call (5:00 PM – 8:00 AM) including the associated weekend; no night visits are required
- Adhere to dress code requirements: nursing staff wear navy blue or navy/white scrubs
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- Minimum 2 years of nursing experience
- Prior home health and/or hospice nursing experience required
- Familiarity with or ability to learn the HCHB documentation system
- Reliable personal vehicle for patient home visits
Preferred Experience / Skills
- Experience working in a home health setting specifically within a rural or county-based service area
- Hospice nursing experience in addition to home health background
