Contract | Days | 4x10s or 5x8s | Every other weekend required | 13 or 26 week assignment
Key Responsibilities
- Provide psychosocial and emotional support to patients and families through a consult-based social work model
- Complete psychosocial assessments and develop person-centered discharge plans in alignment with applicable policies and regulations
- Deliver brief clinical interventions, assist with resource navigation, and coordinate care across the care team
- Screen for suicidality and engage in safety planning as clinically appropriate
- Complete mandatory reports of harm in accordance with state statutes and coordinate with outside entities
- Collaborate closely with RN Case Managers and the broader care team to facilitate safe and ethical discharge planning; may float to like units as needed
Required Qualifications
- Active Alaska LMSW license required</strong>; candidates without a license in hand may apply for an Alaska temporary LMSW license
- Master's degree in Social Work from a CSWE-accredited program
- Minimum 2 years of experience required; acute care experience is required
- Proficiency with Epic or comparable electronic health record system preferred
- Must commit to working 3 of the 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Must live at least 50 miles from the facility to be eligible; local candidates are not accepted
Preferred Experience / Skills
- Experience working within an inpatient hospital setting, including pediatric, NICU, PICU, maternity, or adult inpatient units
- Familiarity with care management social work models and interdisciplinary care team collaboration
- Experience with safety planning, harm reporting, and coordination with external community resources
