Key Responsibilities

• Provide compassionate end-of-life care in patients' homes, ensuring physical and emotional comfort.
• Deliver expert pain and symptom management to support patient well-being.
• Collaborate with interdisciplinary teams, physicians, and community providers to optimize care quality.
• Case manage patients requiring Oasis documentation and oversight, ensuring compliance and quality metrics.
• Offer education and support to patients and families navigating hospice care needs.

Required Qualifications

• Valid Registered Nurse (RN) license.
• Home health and hospice care experience.
• Oasis documentation experience is a must.
• Strong clinical assessment and care planning skills.
• Ability to work independently in a home setting.

Preferred Experience / Skills

• Travel experience in home care settings.
• Case management skills with a focus on Oasis documentation.
• Excellent interpersonal and communication skills.
