Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Medford, OR and surrounding areas
• Conduct patient evaluations and develop individualized treatment plans in a 1:1 care setting
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC and OASIS in an accurate and timely manner
• Manage a caseload with efficient scheduling to minimize drive time across the service area, with occasional travel up to 50+ miles

Required Qualifications

• Minimum of 1 year of skilled home health physical therapy experience required; first-time home health travelers will not be considered
• Active or pending Physical Therapist license in the state of Oregon
• Basic Cardiac Life Support (BCLS) certification required
• Proficiency with OASIS documentation is required
• Must have reliable personal transportation
• COVID-19 primary vaccination series plus at least one booster required

Preferred Experience / Skills

• Experience charting in EPIC
• Familiarity with productivity-based home health models and point expectations
• Comfort traveling across a broad geographic service area
