Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaint in accordance with RN scope of practice
• Develop and implement individualized plans of care, determining desired outcomes in coordination with patients, families, and the inter-professional team
• Recognize, respond to, and communicate changes in patient condition; initiate actions to alleviate stress and discomfort
• Delegate tasks appropriately and monitor accuracy and completeness of delegated patient care activities performed by other team members
• Facilitate patient discharge processes including discharge teaching, execution of discharge orders, and coordination with Case Management
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification required
• Associate Degree in Nursing (ADN or ASN) required
• Ability to perform age-specific patient safety assessments and implement appropriate safety measures
• Effective verbal and written communication skills with the ability to collaborate across care team disciplines
• Proficiency with or ability to be trained on computer systems and clinical applications

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting
• Bachelor of Science in Nursing (BSN)
