Key Responsibilities
- Perform comprehensive assessments of assigned patients, including OASIS and PT evaluations, encompassing psychosocial, educational, safety, and risk factor parameters in accordance with federal and state regulations and APTA guidelines
- Develop and implement individualized plans of care with patient-specific goals, providing skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
- Coordinate patient care as Care Manager and/or team member with physicians, nurses, caregivers, and other rehabilitation services to ensure optimal outcomes
- Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
- Complete OASIS time points, clinical documentation, and billing/charging paperwork accurately and in compliance with agency, state, and federal guidelines
- Actively monitor and manage patient visit schedules to maximize productivity, communicating changes to schedulers and supervisors in a timely manner
Required Qualifications
- Active Physical Therapist license in the state of Vermont (or eligibility to obtain)
- Knowledge of and ability to comply with federal and state home health regulations, including OASIS documentation requirements
- Proficiency in completing required agency, state, and federal forms (e.g., HHABN, Medicare Notice of Non-Coverage)
- Ability to perform patient-related health assessments and procedures, including wound care and staple/suture removal
Preferred Experience / Skills
- Prior experience in a home health setting with OASIS documentation
- Familiarity with McKesson home health software processes
- Experience supervising PTAs, PT students, or home care aides
- Background in quality improvement collaboration and peer review participation
