Contract | Days | Monday–Friday, 8:00 AM–5:00 PM with holiday coverage as needed

Key Responsibilities

• Conduct physical therapy evaluations and treatments during in-home patient visits
• Complete OASIS assessments accurately and in a timely manner
• Travel between patient homes to deliver care, managing a dynamic daily schedule
• Adapt to schedule changes and adjust caseload as needed in a home health setting
• Provide care within hospice contexts as required
• Document patient care using the HealthWyse EMR system

Required Qualifications

• Active Physical Therapist (PT) license in the state of Vermont
• OASIS experience required
• Home health experience required
• Hospice experience required
• Demonstrated experience traveling between patient locations for home visits
• Ability to manage and adapt to frequent schedule changes
• Must be willing to undergo a facility-administered background check

Preferred Experience / Skills

• Prior experience with the HealthWyse EMR system
• Strong organizational and time-management skills in a field-based care environment
