Contract | Full-Time | Days | Monday – Friday, 8:00 AM – 5:00 PM | Some weekend coverage and emergency on-call required | Onsite
Key Responsibilities
- Conduct home visits by traveling from patient to patient within the assigned service area
- Complete and document OASIS (Outcome and Assessment Information Set) assessments accurately and in a timely manner
- Provide skilled nursing care in a home health and hospice setting
- Manage and adapt to changes in daily scheduling as patient needs evolve
- Utilize the HealthWyse (Careficient) EMR system for documentation and care coordination
- Participate in emergency on-call rotation as scheduled
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- OASIS experience required
- Home health and hospice experience required
- Experience traveling between patient homes for scheduled visits
- Valid driver's license in good standing
- BLS certification required
Preferred Experience / Skills
- Familiarity with HealthWyse (Careficient) EMR system
- Strong ability to independently manage a flexible, patient-driven schedule
