Key Responsibilities

  • Evaluate patients' need for physical therapy and develop appropriate, individualized treatment plans
  • Carry out treatment programs in accordance with agency policies and professional standards of care
  • Communicate and coordinate treatment activities with interdisciplinary team members and patients
  • Document evaluations, treatment goals, and objectives accurately and in a timely manner
  • Educate patients and family members on the rationale for treatments and procedures

Required Qualifications

  • Bachelor of Science in Physical Therapy
  • Must hold an active Physical Therapist license in Vermont and New Hampshire, or an unencumbered compact license
  • Minimum of one year of experience as a Physical Therapist in a home health setting

Preferred Experience / Skills

  • Experience working across multiple service territories or geographic regions
  • Familiarity with hospice and rehabilitation care in a community-based setting
