Contract | Days, 5x8 Hours (8:00–16:30)

Key Responsibilities

• Provide speech-language pathology services to patients in a home health setting
• Complete and maintain OASIS assessments in accordance with home health requirements
• Meet productivity expectations of 6 points per day or 30 points per week
• Document patient care accurately and in a timely manner
• Collaborate with the interdisciplinary care team to develop and implement individualized treatment plans

Required Qualifications

• Active Speech Language Pathologist license; Virginia license required to be in hand at submittal or proof of compact license
• Prior home health experience required
• Proficiency with OASIS documentation required
• Specialty experience within the last year required
• One supervisory reference required at time of submittal
• Local candidates are welcome and accepted at the same rate

Preferred Experience / Skills

• Experience meeting home health productivity standards in a fast-paced environment
• Strong interpersonal and communication skills for patient and team collaboration
